MARLTON, N.J., Aug. 11, 2015 /PRNewswire/ -- Continuum Health Alliance (Continuum) and Kryptiq, market leaders in population health management, have formed a strategic relationship to deliver a comprehensive, turn-key solution for Chronic Care Management (CCM), enabling providers to confidently outsource CCM and participate in new value-based incentive and reimbursement programs. Continuum is uniquely positioned to provide clinical and non-clinical practice support on behalf of physicians given the company's legacy as a leading management service organization.
When the Centers for Medicare and Medicaid (CMS) announced it would begin reimbursing Medicare physicians for comprehensive Chronic Care Management (CCM) services under CPT code 99490, it established a significant new revenue stream for providers. However, meeting CCM reimbursement requirements is often challenging. Providers must use a certified EHR, obtain and manage patient consent, deliver five core care management services, and provide at least 20 minutes of follow-up outside of the office (i.e., non-face time care) in conjunction with the CCM program. The combined offering leverages Kryptiq's CCM application, which, according to IDC Health Insights, is the industry's only complete technology solution for CCM. Continuum will bundle Kryptiq CCM in a services wrapper that includes telephonic coaching and call center support.
"We believe physicians will need additional help beyond technology to meet the growing demands of eligible Medicare beneficiaries with two or more chronic conditions. For many providers, success will hinge on the ability to leverage tools and outsource the care management to experts to make this a viable opportunity," said Nicole Bradberry, Chief Product Officer at Continuum. "Our proven track record in delivering the care component combined with Kryptiq's cloud-based, EHR-autonomous care coordination application, creates an end-to-end solution that benefits patients and helps physicians realize new revenue."
The Department of Health and Human Services (HHS) recently announced that it has begun to accelerate the transformation of the healthcare delivery system by more aggressively employing new incentives to motivate higher quality care, increasingly tying value to payment through alternative channels, and supporting emerging care team models. Initiatives like Medicare's chronic care management program were specifically designed to support this transformation and serve as a bridge to value-based care.
"The strategic alliance with Continuum will enable healthcare delivery organizations to pursue population health initiatives and close priority gaps in care with a complete solution that includes both software and services," said Malcolm Costello, Kryptiq Senior Vice President of Product Strategy. "Many provider groups don't have the necessary resources in-house to practice team based care. By taking advantage of externally resourced care coordinators and nurse navigators connected across a common technology platform, they can fulfill the goals of the Triple Aim by aligning evidence-based practices with individual needs utilizing the most affordable resources."
