This dedicated week recognizes the advancements that have been made in patient safety, while acknowledging the challenges that remain. This year's theme is "Patient Safety 7/365: 7 days of recognition, 365 days of commitment to safe care." This is an important reminder that providing safe patient care requires unwavering commitment and awareness throughout the year.
Patient safety is one of the nation's most pressing healthcare challenges. According to the 1999 landmark report by the Institute of Medicine, To Err Is Human: Building a Safer Health System, it is estimated that as many as 98,000 people die in U.S. hospitals each year as a result of medical error. Many believe that the actual number of deaths is higher, and when combined with other non-fatal adverse events, it is clear this is a critical public health issue.
A key component to advancing patient safety is proper hand hygiene, recognized as the number one way to prevent healthcare-associated infections (HAIs), yet the typical rate of hand hygiene compliance among healthcare workers is estimated to be only 40 to 50 percent. HAIs account for nearly 2 million infections annually in the U.S.
