Leading EHR vendors saw some of the greatest market momentum and analytics vendors are also introducing their own care management solutions that are being well-received in the market. Care management vendors are now starting to align around a core care management process: Identify patients, create care plans, communicate care plan goals and tasks to patients, monitor patient progress, and alter care plans as needed. Across the market, vendors are differentiating in the more nuanced parts of the process: How patients are identified, where care plans come from, how well patients are engaged, and how much of the care management process can be automated.
In last year's seminal report on this market, vendors were focused on comprehensive and longitudinal care coordination. This has fallen out of favor with provider organizations due to costs and evasive return on investment. Providers are now focusing more directly on episodic programs directly tied to Medicare reimbursements, such as 30-day readmission penalties or 90-day bundled payment programs.
To address the need for episodic care solutions, providers are beginning to replace legacy solutions focused solely on closing care gaps with those that take a broader approach to population health management and embed the principles of PHM into care management workflows. Rather than selecting all-in-one solutions or care coordination solutions, providers instead are looking at solutions that align with their provider-, payer-, or patient-centric approach to care management.
According to Matt Guldin, Chilmark analyst and co-author of the report, "Provider-led care management adoption continues to increase although it has not yet reached a tipping point. As these programs begin to mature, we are seeing HCOs trying to more closely align their care management workflows to support their particular specific PHM initiatives. As these PHM initiatives continue to scale and expand, we expect these solutions to emphasize greater care coordination by supporting additional workflows that involve different care settings and incorporating non-clinical resources. Efforts to support more longitudinal care management are in the early phases though, and we expect these changes to gradually emerge over the next 3 to 5 years."
