Medical Home's Role in Reforming Health Care Delivery is Central Theme of Patient-Centered Primary Care Collaborative National Summit

WASHINGTON, Oct. 6 /PRNewswire-USNewswire/ -- The nation's top thought leadership and advocates for the patient centered medical home (PCMH) will convene a national meeting in Washington, DC Oct. 22 to unite around the Summit's goal to build, expand and support a fundamentally improved model for health care delivery.
As the debate surrounding health care reform nears a vote in Congress, the Patient-Centered Primary Care Collaborative (PCPCC) will bring together policymakers, academics and medical home pioneers for the one-day event. The PCPCC is a coalition of more than 600 organizations representing the nation's business leaders, consumer groups, organizations representing primary care physicians and other health care stakeholders. The PCMH movement aims to partner every American with a primary health care provider who is responsible and accountable for ongoing care and care coordination.
Congresswoman Allyson Schwartz (D. Pa.) will offer the keynote address. National Economic Council Special Assistant to the President Robert Kocher, MD, will be among participants in a panel discussion of the role of the PCMH in health reform. Two new PCPCC publications will be introduced at the Summit. Across the nation, dozens of patient centered medical homes are already organizing or operating at the local, regional and state levels.
