GLENWOOD, Md., Aug. 4, 2015 /PRNewswire/ -- Healthcare Interactive Inc., a health benefits technology firm, has just launched a new patient-centered medical plan called Habeo Health in partnership with Baltimore-area hospital network GBMC HealthCare and Group Benefit Services Inc., an employee benefits administrator.
The plan is at the forefront of a growing trend among health systems to use "patient-centered medical homes" to improve healthcare quality and reduce costs.

They'll also be able to communicate with their care team and access their health information through a secure, patient-focused portal.
