ACMG of Florida Selects New Population Health Management Solutions Vendor
PISCATAWAY, N.J., June 9, 2015 /PRNewswire/ -- HealthEC®, a leader in population health management solutions, has announced that Accountable Care Medical Group (ACMG) has just chosen to implement their population health management solution. After evaluating more than 10 vendors, ACMG chose HealthEC®'s suite of modules, which cover every aspect of healthcare management. The solution will cover 43 different medical facilities and groups, encompassing 7 laboratories, a number of rehabilitation centers, urgent care centers, home health agencies, physician and nursing facilities, medical supply companies, etc., and over 9,000 patients.
The HealthEC® population health management suite will provide ACMG with a broad solution which includes patient care plans, assorted analytics on care intelligence, identification and management of gaps in care (population stratification), quality reporting measures for GPRO, population benchmarking and payment features, disease, at-risk and wellness tracking, and quality scorecards. The solution will also provide drill-down views on population, sub-populations, geography, date ranges, CPT codes, test, utilization and patient demographics.
ACMG needed a new solution provider: their current vendor was not able to effectively consume and aggregate the various types of claims data from all the different sources they managed. They were seeking a solution that would allow them to integrate, from beginning to end, health data from all of their different types of ACO members, ranging from clinical data, pharmacy and lab data, claims data, medical supply data, etc., as well as patient data, including chronic care patients and patients with other specific health risks, in order to optimize results and raise the quality of life for their covered population. ACMG's main goal was to find a solution that would enable them to ensure that all doctors, hospitals, and other healthcare providers are able to offer the best care possible, by having information from all sources that is coordinated, timely, and easily accessible.
