BOSTON, March 27, 2019 /PRNewswire/ -- Avant-garde Health and Titanium Healthcare, LLC have partnered to enable health systems to improve patient care, as part of the Centers for Medicare & Medicaid Services' (CMS') Bundled Payments for Care Improvement Advanced (BPCI Advanced) initiative. Catalyze^TM, the cross-continuum joint offering, is a turnkey solution that combines the power of the two companies' bundled payment expertise by marrying consulting and software with clinical services.
