NEW YORK, Dec. 4, 2018 /PRNewswire/ -- Brooks Rehabilitation Home Health has selected MetTel's telehealth solution for IoT-connected Remote Patient Monitoring powered by Health Recovery Solutions (HRS), to reduce re-hospitalizations among high risk patients. The partnership leverages the strengths of MetTel's communications capabilities with HRS's clinical software expertise to provide a complete solution for patient monitoring.
Jacksonville, FL-based Brooks Rehabilitation Home Health made the decision to launch a telehealth program with MetTel due to their extensive mobility coverage and the ease of use of HRS software for geriatric patients. The team at Brooks is excited about the ability to use HRS telehealth to enhance communication between patients and caregivers.
Patients placed on the telehealth program at Brooks Rehabilitation Home Health are provided with 4G tablets that will connect to the strongest available signal of any major US wireless carrier. These tablets are pre-loaded with the HRS software and paired with disease specific Bluetooth biometric devices that allow patients to take their blood pressure, weight, heart rate, and more. The patient data automatically transmits to the tablets and is electronically delivered to the patient's clinician, who can identify trends and adjust the treatment plan accordingly.
The HRS telehealth software also encourages patient participation and self-management with daily medication reminders and disease specific educational content. Additionally, patients can communicate with their clinicians via the tablet's text, call, and video chat features.
