CenseoHealth And Advance Health Announce Investment By New Mountain Capital

DALLAS, CHANTILLY, Va., and NEW YORK, Dec. 7, 2017 /PRNewswire/ -- CenseoHealth ("Censeo") and Advance Health ("Advance"), technology-enabled payer service providers, today announced an investment and recapitalization from New Mountain Capital, LLC ("New Mountain"), a growth-oriented investment firm that currently manages over $20 billion in assets. New Mountain will acquire a majority ownership interest in both Censeo and Advance Health. The transaction is currently anticipated to close before the year end 2017, subject to customary closing conditions. Additional terms of the transaction were not disclosed.

The combined company, through a deep network of physicians and nurse practitioners, will provide technology-enabled in-home health assessments nationwide. In addition, the company will provide high acuity health plan members with a range of chronic care management and care coordination services.
