DENVER, June 19, 2014 /PRNewswire/ -- Today, Centura Health's Colorado Accountable Care (CAC) announced it is leveraging the CafeWell Health Optimization Platform, developed by Welltok, to support the health and wellness of its senior population beyond the clinical setting. CAC joins a growing number of accountable care organizations (ACOs) creating innovative ways to partner with consumers to support healthy lifestyles with the goal of delivering better outcomes.
