CINCINNATI, April 4, 2017 /PRNewswire/ -- CommuniCare Health Services welcomes two new healthcare centers to its family in Missouri, increasing its ability to offer specialized post-acute care to residents most in need.

As of April 1, Garden Valley Nursing and Rehabilitation Center and Maple Wood Care Center became a part of CommuniCare. The facility names and locations are as follows:

• Maple Wood Healthcare Center – 724 NE 79^th Street, Kansas City, MO 64118
• Garden Valley Healthcare Center – 8575 N. Granby Avenue, Kansas City, MO 64154

The recent additions increased the total number of healthcare centers operated by CommuniCare to 51 in five states, Ohio, Maryland, Pennsylvania, West Virginia and Missouri.
