CLEVELAND, Sept. 24, 2014 /PRNewswire/ -- COMS Interactive, LLC (also known as "Clinical Outcomes Management Systems" or "COMS") has announced the launch of a continuum of care portal series. The new product offering will work in conjunction with the company's disease management product suite, Daylight IQ™, and the recently acquired Directed Medical Systems product offerings. The Directed Medical Systems team are considered subject matter experts in Advance Care Directives and End of Life care solutions.
Daylight IQ™, a Software as a Service (SaaS) product, is currently being utilized by individual Skilled Nursing Facilities, large chains and Home Care entities throughout the US. The intention of this most recent product extension is to more efficiently connect the healthcare continuum by providing information, access and more active participation by the entire clinical team. The care team includes Hospitals, Managed Care Organizations (MCOs), Accountable Care Organizations (ACOs), Home Care agencies and various Post-Acute providers.
"COMS Interactive is at the forefront of offering products that directly improve clinical outcomes in Skilled Nursing," noted Dr. Josh Luke, Hospital CEO and Founder of the National Readmission Prevention Collaborative. "The concept of offering a care portal series connecting all points in the continuum is perfectly aligned with evolving healthcare needs."
The COMS clinical portal series is part of the Daylight IQ™ Enterprise Edition product offering and provides real-time disease management information and important outcomes-related reports for caregivers throughout the continuum. Care specialists, including Physicians, Nurses, Pharmacists, and Rehabilitation experts, can participate in care at the bedside and/or remotely by leveraging appropriate portals. Clinical management can provide quality assurance, performance improvements and clinical intervention on a more timely basis. The expanded Daylight IQ™ solution includes a dynamically-created portal dashboard, which incorporates a variety of benchmarking reports and clinical views for a single patient or an aggregate of patients across the country.
The Daylight IQ™ product suite facilitates improving quality of care by closely tracking changes in condition and initiating appropriate clinical interventions. Key components of the product suite include an Automated Disease Care Guide Library, a Dynamic Nursing Assessment and Compare Module, all of which work in conjunction with each patient's disease, medication and nutrition profile. Utilizing Daylight IQ™ reduces avoidable 30-day hospital readmissions by 50% and premature mortality by more than 60%.
"We're excited to bring this product extension to the healthcare market," stated Edward J. Tromczynski, Chief Executive Officer, COMS Interactive. "With a focus on Managed Care, the COMS technical platform was designed to support patients through each segment of the continuum, with an emphasis on providing appropriate care and reducing unnecessary hospital readmissions."
Disease management experts, COMS is engaged with leading healthcare organizations in over 40 states to reduce unnecessary hospital readmissions. The US population is aging exponentially, with the number of persons over age 65 doubling to an estimated 70 million by 2030. Notably, 10,000 people turn 65 in the US every day, expanding the country's largest growing demographic segment. Although this aging "Baby Boomer" contingent doesn't necessarily require extended long-term care, competent Post-Acute care is a focal point for the Centers for Medicare and Medicaid Services, Bundled Payment programs, and for MCOs and ACOs.
