Desert Oasis Healthcare is collaborating with Conversio Health to offer pharmacotherapy management and care coordination services to respiratory patients
PALM SPRINGS, Calif., Sept. 26, 2016 /PRNewswire/ -- Desert Oasis Healthcare, an IPA and Medical Group affiliated with Heritage Provider Networks, is partnering with Conversio Health to improve the health and outcomes of respiratory patients.
Desert Oasis Healthcare announced on Sept. 23 the introduction of a new program for chronic obstructive pulmonary disease (COPD) patients. Patients will receive pharmacological treatments with supportive care coordination to help address COPD exacerbations and symptoms. In collaboration with Conversio Health, patients will be offered respiratory medication planning provided by licensed pharmacists and telephonic coaching from a multi-disciplinary team.
