PALM SPRINGS, Calif., Sept. 26, 2016 /PRNewswire/ -- Desert Oasis Healthcare, an IPA and Medical Group affiliated with Heritage Provider Networks, is partnering with Conversio Health to improve the health and outcomes of respiratory patients.
Desert Oasis Healthcare announced on Sept. 23 the introduction of a new program for chronic obstructive pulmonary disease (COPD) patients. Patients will receive pharmacological treatments with supportive care coordination to help address COPD exacerbations and symptoms. In collaboration with Conversio Health, patients will be offered respiratory medication planning provided by licensed pharmacists and telephonic coaching from a multi-disciplinary team.
"COPD is a progressive disease that reduces lung function and increases a feeling of breathlessness. While COPD symptoms and treatments vary, medications are the most common intervention used to limit exacerbations that decrease quality of life," said Brian Hodgkins, Pharm D., Executive VP of Clinical Operations & Heritage California ACO. "Desert Oasis Healthcare is committed to providing the highest quality of medical care to the residents of the Desert community. That includes partnering with organizations like Conversio Health to continually advance the care of our respiratory patients."
Conversio Health's program is ongoing for the length of the patient's enrollment with Desert Oasis Healthcare and is expected to launch October 2016. The remote care coordination from Conversio's team of pharmacists, nurses, respiratory therapists and nutritionists, provides a holistic approach to delivering personalized medicine.
"More than 13 million people in the U.S. suffer from COPD and this disease causes serious long-term disability and early death. While there is no cure, there are steps that can be taken to improve health status and prevent recurring exacerbations," said Kirk Clove, Chief Executive Officer for Conversio Health. "We're proud to join forces with the Desert Oasis Healthcare team to help patients address the COPD issues that directly impact their daily life."
