Developmental Disability Providers Use Therap's Software to Receive Individual Admit, Discharge and Transfer Information

WATERBURY, Conn., Jan. 31, 2017 /PRNewswire/ -- Therap has released an HIE tool that allows for direct information exchange between provider agencies and trading partners i.e hospitals or healthcare providers that they work with to receive the admit, discharge and transfer Information of  individuals. With the HIE too, Therap users can link individuals in the hospital system to individuals on Therap to receive ADT messages. Once linked, agencies will be able to see information including An individual's demographics, hospital visit information, observation results, diagnoses and allergies. ADT messages can then be used to update an individual's medical history and translates on to related health records.

While messages are received directly from hospitals, Therap users have flexibility as to how the information is managed and recorded on the Therap system. Currently, agencies across the country who are providing services to individuals with intellectual and developmental disabilities use Therap for their daily documentation needs. Once the provider agency receives the ADT message regarding an individual, the information may then be acknowledged and imported to the the Individual History forms as a new medical history for the individual. Availability of these information in one specific location helps caregivers obtain an overview of an individual's historical details, allowing them to make decisions accordingly.

Additionally, the Individual History tool on Therap allows support professionals to keep record of admit, discharge and transfer information and update the medical history information according to the received ADT messages. The Individual History module enables users to access and record historical information of individuals they support. Users can keep track of complaints, history of illnesses, family history, reviews, status and much more while accessing necessary medical information from one form.
