EClinCloud Analysis of 1,715 FDA Inspection Citations: A Complete Trial Master File Is Not a Reconstructable Trial

Across six fiscal years of FDA BIMO inspection data, protocol-compliance, informed-consent and case-history obligations account for 46.2% of citation occurrences — in the company's reading, the clearest signal that inspection readiness should focus on whether the trial record can reconstruct what happened, not on how many documents were stored.

Across 1,715 citation occurrences in the agency's published inspection-observation workbooks, obligations covering protocol compliance, informed consent and investigator case histories together account for 793 occurrences, or 46.2%. In the company's reading, that concentration makes protocol adherence and trial records the first places FDA inspection readiness should look.

The report is explicit about what those figures measure: cited obligations, not defect rates or inspection outcomes, summed from the frequency fields of FDA's own workbooks (August 2026 snapshot) with every inclusion rule declared. Its central argument follows from that discipline. A trial file is not demonstrated by how many documents it stores: "A trial file becomes useful when an authorized reviewer can connect a decision to the evidence available at the time." Completeness percentages depend entirely on their denominator — in the report's worked illustration, the same hypothetical file scores 95.0% complete against records that are due and applicable, and 76.0% against the raw catalog — and "neither 95% completeness nor a particular sampled pass rate establishes regulatory acceptability." The practical conclusion: read any completeness figure together with its denominator, and treat a high percentage as the starting point for quality review, not its end.

Retention adds a second layer that storage alone cannot solve. Under EU Clinical Trials Regulation Article 58, the trial master file must generally be kept for at least 25 years after the end of a trial, and the retention clock starts from a legal trigger — approval, discontinuation or completion — that itself must be evidenced. As the report puts it, "guessing a date to complete a dashboard can create a false destruction instruction."

In place of document counting, the report proposes — as the company's own framework, not a regulatory checklist — five questions a TMF quality review should answer for any record:

• Applicability and ownership: does this record apply to this trial, and who is responsible for it?
• Identity and integrity: does it belong to the right trial, complete and with verifiable origin?

• Relationships and decisions: can it be connected to the approvals, actions and decisions it supports?
• Retrieval and preservation: can an authorized user find and understand it throughout the retention period?
