Dr. Ho Cao Vu has incorporated ultrasonic surgical technology into breast augmentation, explantation and reconstructive procedures as part of his surgical practice. During selected breast explant surgery, ultrasonic surgical instruments may be used to support tissue dissection and hemostasis while surgeons assess the implant pocket and surrounding tissue to determine an appropriate surgical plan for each patient.

Breast implant removal may be considered for a range of clinical or aesthetic reasons, including implant rupture, capsular contracture, implant displacement, symmastia, bottoming out, waterfall deformity, recurrent ptosis following breast augmentation, changes in breast shape, or a patient's decision to remove or replace existing implants.

The appropriate surgical approach varies depending on the condition of the implant, surrounding tissue, implant pocket and the patient's overall health.

Following removal of an existing implant, surgeons may assess the implant pocket and surrounding tissue to determine whether additional procedures are required. Depending on clinical findings, these may include capsulectomy, implant-pocket revision, tissue sampling or preparation for implant replacement.

"Breast implant removal should be approached as an individualized surgical procedure rather than a standardized process," said Dr. Ho Cao Vu. "Evaluation of the implant, surrounding tissue and implant pocket helps determine the appropriate surgical plan for each case."

The procedure may also incorporate laryngeal mask airway (LMA) anesthesia in appropriately selected cases, subject to assessment by the anesthesia and surgical teams. The choice of anesthesia and surgical technique depends on the complexity of the procedure, expected operating time, the patient's medical condition and other clinical considerations.

Breast implant explantation is a surgical procedure and carries potential risks including bleeding, infection, hematoma, seroma, scarring and anesthesia-related complications. Pre-operative assessment and post-operative monitoring therefore form part of the overall treatment process.

More complex cases, including suspected implant rupture, significant capsular contracture, intra-pocket bleeding or abnormal tissue findings, may require additional investigation or intervention.
