FDA Inspection Data Reveals a New Compliance Blind Spot for Medical Device Makers

As the FDA deploys AI to analyze submissions at scale, the first QMSR inspection cycle shows problems are spreading across entire quality systems, not just single departments

Rather than treating these as separate issues, investigators are tracing a single thread of risk across a manufacturer's complaint, CAPA, risk, and design systems simultaneously — meaning a gap discovered in one process area is now far more likely to trigger an audit of related processes that were once reviewed independently.

Organizations that still rely on disconnected systems (manually cross-referencing paper records, spreadsheets, and siloed software) are structurally more exposed than ever to this new inspection approach, regardless of how compliant any single process appears on its own.

AI is now working on both sides of the inspection

Compounding this shift is the FDA's own adoption of artificial intelligence. In June 2025, the agency launched ELSA (Evaluate, Learn, and Streamline Activities), an agency-wide AI platform built to analyze submissions at scale and identify compliance patterns across whole industries. The same pattern-recognition capability that helps the FDA spot industry-wide trends can also flag company-specific risks far faster than in previous inspection cycles, collapsing a timeline that once took years down to a single inspection.

Meanwhile, regulators are still debating how far AI can go inside the manufacturing process itself. The EU's draft Annex 22 on data integrity currently excludes generative AI and large language models from critical GMP applications entirely, permitting them only in non-critical contexts with qualified human oversight. However, that could change soon. The European Medicines Agency convened a workshop in May 2026 specifically to evaluate whether dynamic, adaptive, and generative AI models could be permitted under defined guardrails — an open regulatory question that could reshape how life sciences manufacturers deploy AI tools in 2026 and beyond.

Data integrity remains the throughline

Underscoring the stakes, the playbook notes that 61% of recently issued FDA warning letters were data integrity-related — a sustained enforcement focus showing no signs of slowing down. The report frames this alongside the "compliance domino effect": a single, unresolved compliance gap that cascades from inspection prep into production delays, recalls, and warning letters, with remediation costs frequently topping tens of millions of dollars.
