The book, co-sponsored by the American Hospital Association and AARP, features contributions from experts in palliative care, patient and family support, public policy, faith and spirituality, and other aspects of the field. A Roadmap for Success guides readers through the maze of advanced illness care, explains what currently happens to people living with a serious, progressive condition, and shows how society can achieve real change.

There are significant gaps between the care people want and the care they actually get," said Bill Novelli, co-chair of C-TAC and a principal author of the book. While end-of-life care is focused on the last six months of life, advanced illness care can extend for a number of years. Organizations, including health payers, health systems, individual clinicians and faith-based groups are beginning to meet the challenge and provide improved care that increases patient satisfaction and is cost-effective. C-TAC's Roadmap for Success brings together scalable, tangible solutions for the professional community to reach every American with advanced illness needs.
