October 30-November 2 in Nashville

DERBY, Conn., Aug. 4, 2011 /PRNewswire-USNewswire/ -- Founded by a patient more than 30 years ago, Planetree has since developed the most comprehensive model of patient-/resident-centered care based upon thousands of focus groups with patients, families and staff. Building upon these core concepts, today Planetree is a collaborative, global community of health care organizations, innovating new solutions in the best interest of patients as well as caregivers.  Many of these innovations will be shared at the 2011 Planetree Annual Conference, October 30-November 2, in Nashville at the Gaylord Opryland.

The Planetree Annual Conference brings together caregivers across the care continuum to not only inspire innovation, but share practical tactics for the implementation of patient- and resident-centered approaches to care. Additionally, several companies and alliance organizations devoted to patient-centered practices participate in the conference, including ESa, Nurture by Steelcase and the Picker Institute, Planetree Annual Conference sponsors. On the first day of the conference, Sunday, October 30, Planetree affiliates and conference hosts St. Mary's Health System, T.J. Samson Community Hospital, Maury Regional Medical Center, VA of the Tennessee Valley and Central Peninsula Hospital will showcase their facilities.

The four-day event will feature several distinguished keynote speakers, including: Benjamin Zander, Azure Antoinette, Dr. Tracy Gaudet, Dr. Peter Pronovost, Tim Sanders, John Foley, and John F. Crowley.

Given the Affordable Care Act's focus on care coordination, transitions of care and reducing readmissions, the importance of patient-/resident-centered approaches has been elevated to priority one, and the conference will feature more than 100 workshops providing a forum to discuss practical patient-centered strategies. Topics include HCAHPS, value based purchasing, patient and family engagement, bedside shift reports, and Patient-Centered Lean.
