ASBURY PARK, N.J., Nov. 15, 2016 /PRNewswire/ -- GoMo Health announces Healthy Cities, extending the availability of its award-winning, evidence-based population health management and mobile care coordination solution, Concierge Care®, for use by cities, states, and government organizations for use in economically disadvantaged communities in North America, Europe, Asia, and Latin America.

Healthy Cities includes programs for pre-admission, discharge and managed long-term care for prevention and to improve the quality of life for their residents. The program is used to scale the efforts of Federally Qualified Health Clinics (FQHCs) and Community Health Clinics (CHC) and addresses conditions including congestive heart failure, hypertension, asthma, COPD, diabetes, maternity, pediatrics and behavioral health.

A 16-month pilot research study conducted through the University Nebraska Medical Center College of Nursing and funded by Blue Cross Blue Shield Nebraska testing a clinical intervention – [GoMo Health®] mobile technology with community health worker reinforcement. The study used a two-group design to test the intervention's impact on reducing rural preterm births. "The pilot found that the intervention was feasible and cost effective, and was well-received. Participants showed active engagement with the intervention. Both groups had statistically significant improvements in patient activation, with the intervention group scoring higher," says Mary E. A Baltimore city initiative with HealthCare Access Maryland (HCAM), designed to use mobility engagement to provide "in-the-moment" lifestyle education to patients and caregivers to reduce unnecessary Emergency Department readmissions.
