CHATTANOOGA, Tenn., Jan. 15, 2014 /PRNewswire/ -- Grace Healthcare ("Grace") a leading long-term care industry organization, has selected Daylight IQ™, the COMS Interactive (also known as Clinical Outcomes Management Systems or COMS) disease management software to facilitate a reduction in avoidable return to hospital rates.
In addition to 42 Skilled Nursing facilities, Grace also offers Assisted Living, Rehabilitation and Alzheimer/Memory Care. The COMS implementation will focus initially on the Grace Skilled Nursing facilities.
The COMS product suite, Daylight IQ™, provides care information at the bedside for clinical teams. The Software as a Service (SaaS) product significantly empowers the entire care team including physicians, nurses, nursing supervisors and nurse aides, and facilitates improved communication between all stakeholders. Key features of the product suite include an Automated Disease Care Guide Library, a Dynamic Nursing Assessment and Compare Module, all of which work in conjunction with each resident's disease profile. An important component of the Daylight IQ™ program is a reduction in avoidable hospital readmissions by more than 50%.
"Grace Healthcare's culture is based upon a simple but powerful concept – the profession of caring for others is much more than a business, it's a mission," noted Mike Roberts, Chief Operating Officer, Grace Healthcare. "Our management and clinical teams are committed to offering the absolute best possible care to our residents, and Daylight IQ™ will further support us in that endeavor."
The Grace team is implementing the Daylight IQ™ solution in a focused effort to further enhance the level of clinical care provided to the residents. The selection of Daylight IQ™ allows Grace to more effectively leverage their participation in Accountable Care Organization (ACO) and Managed Care Organization (MCO) relationships. In combination, these efforts will provide coordinated high quality clinical care at the right time, while avoiding unnecessary duplication of services and preventing medical errors.
"Grace is clearly a recognized thought leader and we are pleased to be working with this extraordinary clinical team," noted Edward J. Tromczynski, Chief Executive Officer, COMS Interactive, LLC. "Given the changing health care environment and our aging population, taking the initiative to improve upon the quality of Skilled care is even more critical than in the past."
Industry data shows that the average nursing home resident has a complicated disease profile, including one primary disease and up to eight secondary diseases or afflictions. Over 70% of re-hospitalizations from a skilled nursing facility are due to a secondary or new disease progression. By providing early detection of an individual's change in condition, Daylight IQ highlights potential problems and offers caregivers the opportunity to respond, preventing further progression of the illness or affliction.
