Health Plan of San Mateo Returns as a Client to Health Integrated
Health Integrated to Provide Individualized Care Plans for CareAdvantage Members
TAMPA, Fla., Sept. 7, 2016 /PRNewswire/ -- Health Integrated® today announced Health Plan of San Mateo (HPSM) will return as a client for individualized care plan (ICP) services for members of its Medicare-Medicaid Plan and Dual Eligible Special Needs Plan. Health Integrated first assisted HPSM with ICP services in 2014, providing care plans for its most vulnerable members. The success of those 2014 plans led HPSM to renew its partnership with Health Integrated. Health Integrated will provide initial and annual care plans while HPSM's care management team continues its ongoing care management work.
Located in Northern California, HPSM is a non-profit health care plan offering coverage and a provider network to San Mateo County's underserved population. HPSM currently serves more than 140,000 County residents.

Health Integrated will develop ICPs for newly enrolled beneficiaries as well as beneficiaries who are due for annual care plan updating. ICPs will include identified problems, goals and interventions to enable HPSM to provide its members with timely and appropriate services.
