Hello Health Announces Patient Satisfaction Survey for Chronic Care Management Solution
Majority of patients with multiple chronic conditions find value in telehealth program
NEW YORK, July 21, 2016 /PRNewswire/ -- Hello Health, a leader in Meaningful Use physician practice solutions, today announced the Company's first patient satisfaction survey demonstrating the perceived value of Medicare's Chronic Care Management (CCM) telehealth program. Of the 103 patient respondents, 86% offered a value of 'very satisfied' or 'satisfied' with the clinical engagement provided by CCM as part of Hello Health Wellbox Solution.
Since January 1, 2015, Medicare has offered CCM 99490 as a way to increase clinical connectivity between patients with multiple chronic conditions and their primary care providers. Hello Health Wellbox supports this initiative by supplying outsourced telehealth solutions and a dedicated team of care coordinators to monitor and fulfill patients' complex health needs. Each month, participating individuals receive a 20-minute telehealth encounter where care coordinators work to determine a sense of medical wellness. During this time, patients are able to express any concerns they may have, while receiving key instructions and insights necessary to keep them healthy and aging in their homes.

Further insights from the survey include:
- 90% of respondents said they were satisfied/very satisfied with their chronic care coordinator;
- 60% of respondents felt their health benefitted from receiving CCM monthly calls;
- 78% of respondents shared that they would recommend CCM to their family and friends.
