LOUIS, Oct. 8, 2025 /PRNewswire/ -- Home State Health is proud to announce its first Foster Care Center of Excellence (FCCoE), in collaboration with Jordan Valley Community Health Center, marking a significant milestone in the organization's commitment to improving the health outcomes and well-being of children and youth in Missouri's foster care system.
This new Center of Excellence, located in Springfield, is designed to deliver trauma-informed, comprehensive, and coordinated care for children in foster care. As part of a statewide initiative, this partnership aligns with Home State Health's mission to transform the health of the community, one person at a time, by addressing the unique needs of this vulnerable population.

The FCCoE provides:
- A "one-stop shop," working closely with caseworkers and families to quickly connect children and youth to behavioral health, physical health, vision and dental services
- Free health exams and completion of a required form by placement providers to obtain a foster care license
- Prioritizes children and youth's 72-hour and 30-day well child exams to meet required deadlines for children in foster care
- Participating in a pilot program to provide transitional services for youth who are stepping down from high acuity settings, with a goal to prevent readmission
- Dedicated trauma-informed staff including community health workers to assist in care planning, coordination, and support services for families and case workers
