Lahey Health Selects Phytel for Population Health Management to Help Drive Patient-Physician Partnerships and Accountable Care

Technology will support Lahey Accountable Care Organization (ACO) through patient engagement, quality improvement, data integration

DALLAS, April 15, 2014 /PRNewswire/ -- Phytel announced today that Lahey Health has selected the company's comprehensive, Web-based population health and population engagement platform to help keep patients healthy, minimize expense and drive better outcomes.

Phytel's technology is used by hundreds of large physician groups and Accountable Care Organizations (ACOs) across the U.S. to enable timely provider-led interventions that ensure patients get the care they need when they need it. Lahey Health launched the Lahey Clinical Performance ACO in 2013 to deliver seamless, high-quality care for Medicare beneficiaries.

Lahey Health selected Phytel based on the company's track record of success and its ability to integrate the data of multiple applications from different vendors. While the solution will be rolled out initially to physicians employed by Lahey Health, the organization expects to eventually offer it, additionally, to affiliated physicians who use a wide variety of electronic health record (EHR) and practice management systems.

Lahey Health will use the Phytel platform to identify patients with gaps in recommended care, empower care managers with automated tools, and engage patients in their care - improving quality and health outcomes.

  • Automated Communication enabling providers to launch patient intervention campaigns targeting select populations using phone, email and text messaging.
