Los Angeles CCTP Earns Renewal with Support from Loopback Analytics

DALLAS, June 9, 2015 /PRNewswire/ -- Los Angeles-based Partners in Care Foundation (Partners) has earned a one-year renewal for two of its community-based care transition programs (CCTP), Westside and Glendale, with the help of patient data and streamlined care coordination workflows enabled by the Loopback Analytics platform.

Partners' Westside CCTP supports patients from three hospitals, representing two hospital systems, while the Glendale CCTP supports patients from three hospitals, representing two systems. A team of more than 20 Partners health coaches provide a variety of care transition services, such as medication inventory assistance, to patients.
