ModifyHealth Expands Executive Team to Accelerate Growth and Impact

New leadership roles further strengthen ModifyHealth's capabilities across operations, technology, and health plan partnerships as the company expands its integrated nutrition and social care platform.

ATLANTA, Sept. 29, 2026 /PRNewswire/ -- ModifyHealth, a national leader in medically tailored meals, nutrition support, and social care navigation, today announced four new senior executive hires: Zachary Clark, as Chief Operating Officer; Joshua Sclar, MD, as Chief Health and Product Officer; Logan Hasson, as Chief Technology Officer; and Addison Giannini, as Executive Vice President, Health Plans.

The appointments represent continued investment in the leadership team as ModifyHealth expands its role as an integrated partner for health plans managing chronic disease, quality performance, and health-related social needs. ModifyHealth supports health plans and social care networks with nationwide nutrition delivery, registered dietitian care, member engagement, and closed-loop social care navigation. Clark will lead operations, overseeing the delivery and growth of ModifyHealth's products and services. He joins from Uber Health, where he served as Global General Manager. Under his leadership, Uber Health grew to serve more than 4,000 healthcare organizations across more than 250 U.S. cities, pioneering the integration of non-emergency medical transportation and health benefits delivery within the Medicare Advantage ecosystem. Clark forged landmark partnerships that enabled Medicare Advantage members to access healthcare benefits through flexible spending accounts, demonstrating his ability to connect complex payer infrastructure to the people who rely on it most.

Sclar, a physician executive, will oversee healthcare strategy and product, connecting ModifyHealth's nutrition programs, dietitian network, and care navigation model to the outcomes health plans care about. Hasson will lead engineering and technology, including CareScape, ModifyHealth's proprietary platform combining member screening, multilingual community health worker outreach, and closed-loop referrals into services. He brings more than a decade of software engineering and technology leadership experience, including as a member of the founding team at Cityblock Health.

Giannini joins as Executive Vice President, Health Plans, with responsibility for expanding ModifyHealth's health plan partnerships and driving growth, with a particular focus on Medicare and national programs. He has extensive experience with healthcare technology and services startups providing solutions to health plans in the US.
