My Health Medical Group named 2013 Patient-Centered Medical Home of the Year, becomes first in San Francisco to earn top NCQA recognition

SAN FRANCISCO, March 6, 2013 /PRNewswire/ -- My Health Medical Group, an affiliate of Brown & Toland Physicians (www.brownandtoland.com), won the 2013 "Patient-Centered Medical Home Practice of the Year" award from the California Academy of Family Physicians at its annual conference March 2 and 3.

My Health Medical Group (www.myhealthmedicalgroup.com) also became the first practice in San Francisco to earn recognition as a "Level 3 Patient-Centered Medical Home" from the National Committee for Quality Assurance (NCQA). Level 3 is the highest recognition possible from NCQA, a nonprofit healthcare accreditation entity that validates practices as patient-centered medical homes using a rigorous set of national industry standards.
