New Publication Highlights That Changes in FSH, Not Estrogen, May Be an Earlier Sign of Perimenopause

Peer-reviewed analysis from Dr. Deanna Minich and six co-authors links rising FSH levels to bone loss, brain changes, and other health shifts years before typical menopause symptoms emerge

WEST VALLEY CITY, Utah, Sept. 22, 2026 /PRNewswire/ -- A new peer-reviewed review published this month in the Journal of Clinical Medicine is challenging one of the most basic assumptions in menopause research: that follicle-stimulating hormone (FSH) is simply a marker of fertility.

The review, led by Dr. Deanna Minich and six co-authors, arrives during Perimenopause Awareness Month and posits that FSH may do far more than signal the end of a woman's reproductive years. Drawing on decades of published research, the authors found that FSH receptors — long thought to exist only in the ovaries — have also been identified in bone, fat tissue, the brain, blood vessels, immune cells, and the kidneys. That distribution, the authors write, raises the question of whether FSH functions as a broader signal of aging across the body, not just a marker of declining ovarian function.

"FSH has been treated as a single-purpose hormone for decades because that's the role it was first discovered playing," Dr. Minich said. "The evidence we pulled together tells a more complicated story — one where this hormone may be worth watching well beyond a fertility test."

Among the review's key findings:

Bone: The strongest evidence in the review ties rising FSH to bone loss independent of estrogen. Citing prior research from the SWAN cohort, the authors note that each doubling of FSH was associated with a 39% greater risk of lumbar spine bone loss within a year, compared with a 10% risk increase for each halving of estradiol — suggesting FSH tracks with skeletal changes even more closely than the hormone traditionally blamed for menopausal bone loss.

Brain: The review cites imaging research on 191 midlife women showing that higher circulating FSH was associated with greater beta-amyloid deposition and lower gray matter volume in brain regions tied to Alzheimer's risk — associations that held up after accounting for age, estrogen, and genetic risk factors.

Metabolism, cardiovascular, and immune health: In one cohort of women aged 30 to 70, higher FSH was associated with less lean muscle, while estradiol and testosterone showed no similar relationship. In animal studies, blocking FSH reduced total body fat and shifted fat tissue toward a more metabolically active type. The authors are careful to note this evidence is less consistent than the bone findings, reflects associations and early-stage research rather than demonstrated effects in humans, and warrants further study.

The review does not propose FSH as a new treatment target, nor does it question the role of estrogen therapy, which the authors note remains the most effective way to lower FSH. Instead, the authors argue that tracking FSH over time, alongside estradiol and other markers, could offer an early signal of endocrine changes — one that current clinical practice largely overlooks.
