Nine new analyses show Novartis' LCZ696 could change course of heart failure for patients

- New data from PARADIGM-HF shows LCZ696 cut incidence of sudden deaths, emergency room visits, hospitalizations, worsening symptoms and need for more intense treatment in HFrEF patients versus enalapril(1,2)

- Patients' and doctors' assessments of disease severity were also significantly better with LCZ696 than enalapril(1)

- Effects on certain heart biomarkers indicate that compared to enalapril, LCZ696 reduced cardiac stress and damage(1)

- Heart failure is highly debilitating and life-threatening(3) - up to half of patients who die from cardiovascular causes will die suddenly when their heart stops pumping, before medical intervention is possible(4)

EAST HANOVER, N.J., Nov. 17, 2014 /PRNewswire/ -- New data on Novartis' investigational medicine, LCZ696, for patients with heart failure with reduced ejection fraction (HFrEF) shows it has the potential to change the course of the disease for patients.1,2 In August 2014, Novartis presented topline results from the landmark PARADIGM-HF study showing LCZ696 was superior to ACE inhibitor enalapril on key endpoints, including significantly reducing the risk of cardiovascular (CV) death or heart failure hospitalization.5

The new analyses being presented for the first time at the American Heart Association Scientific Sessions 2014, with a paper being simultaneously published in Circulation, show that versus enalapril, LCZ696 significantly:1,2

  • reduced the risk of dying suddenly by 20% –  in HFrEF patients, 45% of CV deaths and 36% of all-cause deaths were sudden6
  • reduced first and subsequent HFrEF hospitalizations by 21% and 23%, respectively
  • reduced hospitalizations for a CV reason or for any reason both by 16%
  • reduced the need for more intense outpatient treatment by 16%
  • reduced emergency room visits because of rapid symptom worsening by 30%

When hospitalized, LCZ696 and enalapril patients remained under care for approximately the same amount of time, but those on LCZ696 had 18% fewer stays in intensive care and were 31% less likely to need IV drugs to help their heart pump. Patients' reports of how well they felt and doctors' assessments of disease severity were also significantly better with LCZ696 than enalapril.1

This therapy offers hope to millions of people living with HFrEF that they can also reduce or slow the decline in their heart function, potentially altering the progression of their disease," said David Epstein, Division Head, Novartis Pharmaceuticals.

Analysis of cardiac biomarkers (NTpro-BNP and troponin), substances that indicate risk and progression of cardiac disease, showed levels were consistently lower with LCZ696 than enalapril, reflecting reduced heart stress and subsequent damage.1

LCZ696, a twice-a-day medicine being investigated for heart failure, acts to enhance the protective neurohormonal systems of the heart (NP system) while simultaneously suppressing the harmful effects of the overactive renin-angiotensin-aldosterone system (RAAS). Currently available medicines for HFrEF only block the harmful effects and mortality remains very high, with up to 50% of patients dying within 5 years of a diagnosis of heart failure.7,8,9

Novartis plans to complete the New Drug Application for LCZ696 with the US Food and Drug Administration (FDA) by the end of 2014.

About the PARADIGM-HF study
PARADIGM-HF is a randomized, double-blind, Phase III study that evaluated the efficacy and safety profile of LCZ696 versus enalapril (a widely studied ACE inhibitor) in 8,442 patients with HFrEF.5 The baseline characteristics showed the patients enrolled were typical HFrEF patients with NYHA Class II-IV heart failure.10 PARADIGM-HF was specifically designed to see if LCZ696 could decrease CV mortality by at least 15% versus enalapril.11 Patients received LCZ696 or enalapril in addition to current best treatment regimen. The primary endpoint was a composite of time to first occurrence of either CV death or heart failure hospitalization, and it is the largest heart failure study ever done.10,11

About LCZ696 in heart failure
LCZ696 is an ARNI (Angiotensin Receptor Neprilysin Inhibitor) and has a unique mode of action which is thought to reduce the strain on the failing heart.8,11

Heart failure is a debilitating and life-threatening disease in which the heart cannot pump enough blood around the body. Symptoms such as breathlessness, fatigue and fluid retention can appear slowly and worsen over time, significantly impacting quality of life.3

Heart failure represents a major and growing health-economic burden that currently exceeds $30 billion in the United States, which accounts for both direct and indirect costs.12 Hospitalizations account for 80% of the $21 billion spent on the direct costs of heart failure.12
