Rainfall Health Announces New Health System Customer to Support CMS TEAM Model Implementation

AI platform will support care coordination and compliance efforts for a participating Florida health system under CMS's Transforming Episode Accountability Model

SAN FRANCISCO, Sept. 2, 2026 /PRNewswire/ -- Rainfall Health, an AI-driven compliance and reimbursement platform for hospitals and medical groups, today announced that Mayo Clinic in Jacksonville, Florida will implement Rainfall's platform to support care coordination and compliance activities associated with the Centers for Medicare & Medicaid Services (CMS) Transforming Episode Accountability Model (TEAM), which launched January 1, 2026.

Rainfall's platform is designed to support hospitals and post-acute care partners by helping coordinate care, monitor patient progress, and automate workflows associated with value-based care models such as TEAM. The platform will be used to support post-acute care coordination and quality reporting efforts for participating patients.

CMS selected 721 hospitals to participate in the first cohort of TEAM, a mandatory payment model focused on improving quality and coordination of care for five high-volume surgical episodes through value-based reimbursement. Under this structure, hospitals that fall short of CMS's cost and quality benchmarks face steep financial penalties. However, hospitals that hit their benchmarks stand to gain the other side of that same equation: reconciliation bonuses built into CMS's incentive structure that can translate into up to $85M in new revenue.
