NEW YORK, Sept. 9, 2015 /PRNewswire/ -- Sense Health is expanding its role in Medicaid care management programs in New York, the second largest state Medicaid program. Sense Health's patient engagement technology enhances a care manager's ability to provide personalized, outcomes-driven support for growing patient caseloads through two-way messaging and care coordination features. As part of this expansion, Sense Health is proud to announce its collaboration with Montefiore Health System to build out Montefiore's mobile patient communication abilities.
"At Sense Health we're highly focused on building technologies to support care management programs for Medicaid patients," said Stan Berkow, Founder and CEO of Sense Health, "We're thrilled to partner with Montefiore as one of the most respected systems in this space."
The partnership was sparked following a successful randomized control trial within a Medicaid patient population at University Behavioral Associates (UBA), which is affiliated with the Montefiore Health System. In the trial, Sense Health demonstrated a significant improvement across key patient engagement metrics, including: self-reported patient adherence to appointments (40% increase), medications (12% increase) and care plan goals (7% increase), compared to patients receiving care management services alone.
"We are deeply committed to constantly evaluating how to best engage with the communities we serve," said Mony Weschler, Senior Director Applications Strategy & Innovation. "Leveraging mobile technology like Sense Health is an important method to help try to ensure our population feels connected to our providers and have measurable improved outcomes as they move across different care settings."
