BIRMINGHAM, Ala., April 24, 2012 /PRNewswire/ -- The National Committee for Quality Assurance (NCQA) recently announced that SuccessEHS client Ruth U. Fertel / Tulane Community Health Center received the highest level of recognition from its 2011 Patient-Centered Medical Home (PCMH) program. Six of Tulane CHC's providers also earned NCQA 2011 PCMH Level 3 recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and integrate patients as active participants in care. These six providers include: Caroline Christopher, MD; Edith Gabor, NP; Christopher Joplin, MD; Eboni Price-Haywood, MD; Michele Simoneaux, MD; and Philip Skelding, MD.
The PCMH care delivery model is designed to strengthen the physician-patient relationship by moving from episodic care to coordinated care and an ongoing relationship with a physician-led care team for wellness and long-term healing. Key domains of the model include improved patient access to care through open scheduling and expanded hours, and strong communication among patients, physicians and staff. Technology plays a key role in achievement.
"SuccessEHS has played a critical role in our ability to deliver high quality health care through the PCMH model by offering practice management and EHR features that enhance care coordination, population health management and care team communication," said Dr. Eboni Price-Haywood, Co-Executive Director at Tulane Community Health Centers. "We recently underwent the renewal of our flagship clinic's (Ruth U. Fertel / Tulane Community Health Center) recognition as a Level 3 PCMH under the new 2011 NCQA standards. During this process, we worked closely with SuccessEHS to develop a PCMH toolkit with software enhancements to help other clients learn how to most effectively use SuccessEHS software to implement the NCQA medical home model of care and to improve care team workflow efficiency. One major lesson we learned through our PCMH transformation is that simply having health information technology does not automatically help one to deliver high quality care; the technology has to be built in a meaningful way in order for clients to use the technology to deliver care in a meaningful way. SuccessEHS's commitment to improving technology and its willingness to partner with clients is an exemplary approach to helping providers improve the overall quality of health care."
