SuccessEHS Releases Patient-Centered Medical Home (PCMH) Toolkit

BIRMINGHAM, Ala., Dec. 15, 2011 /PRNewswire/ -- SuccessEHS announces the release of the new Patient-Centered Medical Home (PCMH) Toolkit, designed to equip clients with a dependable roadmap to NCQA 2011 PCMH recognition, which incorporates and expands upon the meaningful use criteria. The release of the Toolkit complements the robust capabilities within the SuccessEHS solution that support the patient-centered care model, and underscores SuccessEHS's continued commitment to its clients' success in the PCMH arena.  

In addition to focusing on patient engagement to move from episodic care to long-term healing, providers within a PCMH gain access to a more extensive and accurate scope of patient data, and can therefore recommend more relevant treatment programs, make more accurate diagnoses, and ultimately improve the quality of care their patients receive while conserving costs. The SuccessEHS PCMH Toolkit provides a comprehensive guide to achieving this patient-centric care delivery model, allowing clients to access full correlating reports, sample policies and procedures, and step-by-step "best-practice" workflows. Since the concept of Patient-Centered Medical Home (PCMH) recognition was first established, SuccessEHS has remained actively involved in initiatives and organizations advancing patient-centered care, including Toolkit collaboration with the ONC Crescent City Beacon Community (CCBC) Project in the Greater New Orleans area. Funded by the Office of the National Coordinator (ONC), CCBC was awarded approximately $13 million over three years, and focuses on reducing the burden of chronic disease by utilizing health IT to improve quality of care for chronic patients in patient-centered medical homes, impact population outcomes for chronic disease, reduce healthcare costs by decreasing preventable emergency department and inpatient visits through better coordination of care for chronic disease patients, and use innovative technologies to engage patients.

The implementation of an electronic health record in the context of a patient-centered medical home is central to providing outcomes-based care. The partnership between CCBC and SuccessEHS has focused on facilitating a community-wide discussion on the PCMH model and the workflow requirements from the primary care practice and EMR vendor perspective, and highlight opportunities for information exchange across the continuum of care. This resulting information has been incorporated into the PCMH Toolkit; use of this toolkit is beneficial to SuccessEHS customers throughout the country, as well as the patients they serve.
