The Geneia Institute Helps Hamilton Health Center Succeed as a Patient-Centered Medical Home

HARRISBURG, Pa., Oct. 3, 2013 /PRNewswire/ -- The Geneia Institute today announced a partnership with the Hamilton Health Center to help it succeed as a patient-centered medical home.  Rooted in the goals of improving the quality and cost of health care for Hamilton's patients, the partnership includes comprehensive education and training for the Center's administrative and clinical staff.

The Hamilton Health Center, a Federally Qualified Health Center that serves the residents of the Greater Harrisburg area with medical, dental and related social services, created a medical home to provide a higher level of coordinated care to its patients.  Medical home patients work closely with a chosen primary care physician, a medical assistant, and a care team to manage and improve their health.
