WATERBURY, Conn., April 18, 2016 /PRNewswire/ -- Therap Services has released an interface to support conflict free case management in states that offer targeted case management and managed care. According to CMS's policies on conflict free case management, provider agencies that offer case management would not be involved in providing direct services or developing person centric service plans. Therap's case management functionalities allow case managers to view data of the individuals that they are supporting across different service providers They can view documentation created by day and residential providers outside of their own agency with the help of a set of access privileges assigned by the State.
The Therap interface comes with a multi purpose design that allows for case managers to view information recorded for individuals they support at other service provider agencies, and also record documentation on individuals within their own managed care organization (MCOs). The case management features allows for case managers to set their own access privileges to sort the data that is relevant to them and unique to the individuals they support. The Therap case management software allows to view person centric data including service plans, individualized notes, incident reports, medication administrations and an assortment of health records ranging from doctor's appointments and scheduled lab tests to immunizations and active allergies.
Therap brings a level of communication and transparency to conflict free case management with the new case management tools. The interface has already seen multiple statewide implementation and is ready to meet the case management needs of other transitioning states who are aligned with the policies of the Centers for Medicare & Medicaid Services (CMS).
