TIRR Memorial Hermann and Belmont Village Senior Living Collaborate to Expand Access to Rehab and Post-Acute Care
HOUSTON, Nov. 30, 2016 /PRNewswire/ -- Houston-based Belmont Village Senior Living and TIRR Memorial Hermann recently came together in agreements that will make rehabilitation and post-acute services more accessible for Houston residents. TIRR Memorial Hermann opened a new integrated physician and therapy practice on the Belmont Village campus at 2909 West Holcombe Boulevard this summer. The outpatient facility offers services for adults in the areas of cancer rehabilitation, lymphedema and limb loss integrated with rehabilitation physician care in a single, coordinated practice location. The facility provides a much-needed therapy option for the West U area and for Belmont Village long-term and transitional residents who need the specific rehab care available at the outpatient center.

Belmont Village also entered into a service agreement in October making Memorial Hermann Health System an in-house provider of home health services and physical, occupational and speech rehab therapies for residents in their two Houston communities, Belmont Village West University at 2929 West Holcombe Blvd, and Belmont Village Hunters Creek at 7667 Woodway Drive. Each community will be served by its own dedicated Memorial Hermann therapy and nursing teams to provide consistency of care.
