ORLANDO, Fla., Aug. 27, 2026 /PRNewswire/ -- UPMC has expanded an AI-native clinical services model in transitional care management to 10 of its hospitals, with one more launching September 1 and seven more on September 22. The program started at a single campus, UPMC Jameson in New Castle, Pennsylvania, and now extends across the system. It runs on ThinkAndor®, Andor Health's AI-native infrastructure, and is operated by Andor's clinical services Psynergy Health team.
Who does the work
The pieces that normally get split between a software vendor and a health system's own staff are combined in the Andor/Psynergy AI-native clinical services suite. Psynergy Health's clinicians see UPMC patients using Andor Health's software, and the work surrounding those visits belongs to the same team: outreach, scheduling, visit completion, clinical judgment, documentation, billing, and compliance. AI runs underneath all of it, sorting which patients need attention first, preparing the clinician ahead of each encounter, and taking on the administrative load that would otherwise take time away from the visit.
Because those pieces are not spread across three organizations, accountability sits in one place. Building the program campus by campus
Psynergy Health's teams and UPMC's clinical leaders have spent the past 18 months getting better at this together by refining the program design and testing changes against what the data showed and what UPMC's own staff were seeing on the floor. Two changes made the biggest difference: a patient education flyer that helps patients understand their own follow-up plan and outreach that is co-branded with UPMC, so patients recognize it is coming from their own hospital. The team also moved the automated check-in up to day 15, catching problems earlier when they are most likely to send a patient back to the hospital.
Two things come out of a stretch like that. The model molds to how each health system actually operates instead of arriving fixed at implementation. And it reaches each new campus already tested against the way UPMC's own teams work.
Closing the gap after discharge
The days right after discharge are where much of the avoidable harm in healthcare happens. Patients lose contact with their care team. Symptoms go unmanaged. People come back to the hospital for something that could have been caught earlier in the week. Most health systems understand this perfectly well and still cannot fix it, because the staffing shortage hits hardest in exactly the roles built to keep patients connected.
Traditionally, those clinicians, often nurses, have spent a meaningful share of their time doing work below the top of their license: working through call lists, making repeated outreach attempts, asking the same initial questions, and documenting who needs follow-up. The constraint is not clinical judgment. It is that too much of a clinician's day gets consumed simply trying to get to the encounter.
Under this program, AI automation changes that operating model. It reaches every enrolled UPMC patient within 48 hours of going home, handles the first pass, and routes the patients who need a live conversation directly to a care coordinator's call list. That is 100% of enrolled patients, including the ones who do not pick up the first time and the ones with no risk score flagged. That reach is only possible because AI, not a coordinator working down a list of one name at a time, is doing the repetitive work required to find the patients who need attention.
What the technology does
Most healthcare AI listens to a visit and drafts a note. ThinkAndor® works differently. It reasons across the patient's full record, discharge summaries, remote monitoring data, and patient-reported outcomes, and surfaces what a clinician should act on before that clinician opens the chart. Transitional care agents built on this infrastructure have cut clinician time on non-clinical tasks by 63% per encounter, and AI triage on monitoring data has removed 82% of non-actionable alerts. Health systems running the model have also seen a 180% increase in patients seen after discharge and a 45% reduction in total cost of care.
