Valence Health Makes Enhancements to Clinical Integration Web Portal
CHICAGO, Feb. 16, 2012 /PRNewswire/ -- Valence Health, a leading provider of clinical integration (CI) and data management solutions, recently launched three primary enhancements to Vision®, its CI web portal. The Vision portal contains a portfolio of reports that enable administrators and providers to access robust analysis and reporting tools, promoting more accurate assessment of patient conditions, treatment compliance, and outcomes. The recent additions to the Vision portal – including an updated Patient Profiler Tool, the Condition Registry Report, and the Patient Outreach Tool – further support population management and provider organizations seeking to deliver accountable care.
The Vision portal has two levels of access: Administrative and Physician. The Administrative View is designed for a CI/Quality Committee or CMO to analyze performance across the Clinically Integrated Network (CIN) for all quality measures as well as at the individual physician level. The Physician View allows each physician to review his/her performance for applicable guidelines and measures, with comparisons to a CIN peer group.
The newly updated Patient Profiler Tool supplies a comprehensive view of the care delivered to the patient by CIN providers. It includes the Action List, which provides both a retrospective and prospective view of the care elements needed by the patient, based on the CIN's clinical guidelines and measures. The Action List also includes an indicator for overdue services and recall dates for services coming due, which enables users to systematically identify and contact patients to ensure services are received in a timely manner, in accordance with evidence-based standards of care.
The Patient Profiler Tool also includes a Condition Registry, which reports all of the conditions identified for the patient, including chronic diseases, along with the most recent dates of service with CIN providers. This gives the physician and care management team a better overall view of the patient's health status and care delivery requirements.
The last new addition to Vision, the Condition Registry Report, allows the user to run predefined registry reports for specific patient populations as well as create custom reports using variables such as Condition, Care Elements, Age, Gender, and Time Periods. This flexible reporting tool supports true population management by identifying patients with chronic conditions who require ongoing disease management or complex care coordination, and readily identifying issues for follow up or intervention.
To support patient communication and follow up, the Patient Outreach Tool has been embedded within the Vision Reports. Using email or letter functionality, the user can outreach to the patient with service reminders or a message to contact the practice. Patients may be contacted individually or an entire registry of patients can be reached with a few simple steps. The Patient Outreach Tool greatly promotes communication efficiency and facilitates more effective management of patient populations.
