Visiting Nurse Service of New York Advances Rose Madden-Baer to SVP Leading New Population Health Management Position

NEW YORK, July 2, 2014 /PRNewswire-USNewswire/ -- The Visiting Nurse Service of New York (VNSNY) today announced the appointment of Rose Madden-Baer to Senior Vice President, Population Health Management. In this newly created position, Dr. Madden-Baer will collaborate with senior leadership in the development and oversight of initiatives that ensure continual advances in patient service quality and best practices across the VNSNY enterprise.

Madden-Baer and her team will focus on ensuring that VNSNY patient care delivered through Provider Services and VNSNY CHOICE Health Plan care coordination efforts are aligned with both the IHI Triple Aim framework and with New York State's Medicaid objectives: improving patient care, quality and satisfaction, improving the health of populations, and reducing the per capita cost of health care.

In her new position, Madden-Baer will collaborate with program and regional leadership to identify, develop and implement strategies designed to overcome system, policy, and/or practice barriers to the highest standards and quality of care. She will report to VNSNY Senior Vice President and Chief Administrative Officer Kevin Rogers, and her primary responsibilities will include development of new evidence-based programs and creation of clinical operations improvement strategies focused on care management and new population health models.

During her tenure at VNSNY, Madden-Baer's innovative accomplishments have brought recognition to the vital importance of home- and community-based health practices in our nation's changing health care landscape. As Vice President of Clinical Operation Strategy and Development at VNSNY, she initiated systems of care that translated patient characteristics into clinical program development, federal and state funding modifications, and cross-continuum partnerships. Her groundbreaking work and research in the development of behavioral health programs and depression screening and interventions has been nationally recognized.

For more than 30 years, Rose Madden-Baer has practiced as both a nurse and nursing leader in a variety of home health, MLTC (Managed Long Term Care), and community-based settings. She is a board certified public health/community health clinical specialist, and holds a Doctorate of Nursing Practice from Duke University School of Nursing. She is certified as a Professional in Healthcare Quality, as a Home Care and Hospice Executive, as an OASIS specialist, and as a Population Health Care Coordinator (Duke University).

Rose Madden-Baer is the recipient of the 2014 VNAA Innovative Leader of the Year Award and was appointed as a Consultant Associate Faculty Professor at the Duke School of Nursing, where she was recently awarded the Distinguished Alumna Award. In 2013, she was honored as a finalist in the Nurse.com (formerly Nursing Spectrum Magazine) Excellence Awards program in the Advancing & Leading the Profession category. She was a finalist in The New York Times Tribute to Nurses in the Innovation category.  Under Madden-Baer's leadership, VNSNY's Behavioral Health program won top honors as ADVANCE for Nurses' Best Nursing Team across their eight-state Northeast region and among the 10 best nursing teams nationally.
