
CHICAGO, April 1, 2026 /PRNewswire/ -- Vitae Health Systems ("Vitae" or the "Company"), a multidisciplinary provider group delivering integrated primary care, behavioral health, and specialty services across the post-acute continuum, announced today a series of senior leadership appointments designed to strengthen the Company's operational infrastructure and accelerate its next phase of growth. These additions reflect Vitae's continued investment in building a best-in-class leadership team capable of furthering the company's mission of delivering excellent care to seniors across the country.
The appointments follow Vitae's partnership with Lorient in 2025, which provided the Company with targeted resources designed to help Vitae scale its integrated care model across skilled nursing and long-term care settings in existing and new markets. With that foundation in place, Vitae has moved deliberately to build out leadership depth across operational, administrative, and value-based care functions.
Leadership Appointments
Isaac Freund has been promoted to Chief Executive Officer. A co-founder of Vitae, Isaac previously served as Chief Operating Officer and has been the driving force behind the Company's evolution from a regional provider group into a multi-state organization. A former nursing home administrator, he has led Vitae's value-based care strategy, established key relationships, and guided the Company through its institutional partnership. Isaac brings a rare combination of clinical setting expertise and executive leadership to the CEO role.
Justin Bhandari has been promoted to Chief Operating Officer. Previously serving as EVP of Operations, Justin has led enterprise-wide operating initiatives and brings extensive experience from leadership roles across provider groups, senior living, and healthcare services organizations. He will oversee day-to-day operations and support the Company's market expansion efforts.
Mary Bogasky has been appointed Vice President, Compliance. A licensed therapist by training, Mary brings significant experience designing and administering compliance programs in complex healthcare environments, including prior leadership roles at Wound Pros and Cerebral. Her background bridges clinical and regulatory perspectives in a way uniquely suited to Vitae's care model.
Caroline Jackson has been appointed Vice President, Human Resources. An experienced HR leader with a track record of building people operations within high-growth, private-equity-backed organizations, including Synapse and Triple Aim Partners, Caroline brings deep expertise in talent acquisition, total rewards design, and organizational development.
Samantha Pasek has been appointed Vice President, Revenue Cycle Management. Samantha brings more than 15 years of revenue cycle operations experience, including leadership at Paragon Health with a focus on technology enablement and scalable billing infrastructure. Her appointment reflects Vitae's continued investment in the financial and operational systems needed to support a growing multi-state footprint.
Investing in Value-Based Care
Value-based care is central to Vitae's clinical and business model. As the Company scales its presence across skilled nursing and long-term care settings, it is making deliberate investments in the infrastructure, talent, and programs designed to support measurable outcomes and payer partnerships built on shared savings and performance accountability.
Vitae is investing in value-based care across several dimensions: expanding care coordination and population health infrastructure, deepening integration between clinical teams and facility partners, and building the payer relationships and analytics capabilities needed to perform in risk-based arrangements. To support this work, the Company has also added dedicated operational leadership, including the appointment of Eric Highley as Director, Value-Based Care Operations. Eric brings more than a decade of experience from UnitedHealthcare and Navvis Healthcare, where he led population health strategy across Medicare Advantage, Medicaid, and commercial programs.
