ZynxCarebook dramatically improves care team collaboration

Enhanced secure messaging capabilities in the ZynxCarebook mobile care coordination solution extend beyond individual communications, empowering interdisciplinary care team members to initiate evidence-based workflows when transitioning patients from the hospital to other care settings. ZynxCarebook is accessible on Apple iOS and Android devices.

ZynxCarebook's host of features improves collaboration and communication among clinicians within a single health facility as well as across multiple care institutions. Among these features is the ability to create cross-continuum, patient-centered care teams; develop virtual huddles to improve collaboration between providers in different care settings; and incorporate evidence-based intervention guidelines into the patient transition care process. Collaborating on and standardizing care helps enhance quality, allowing healthcare organizations to create safe and effective care transitions that improve patient outcomes, reduce readmissions and ensure consistent compliance.

Knowledge Analyzer promotes best evidence-based decisions

The sheer volume of clinical evidence has made it impossible for even the most dedicated clinician to keep current with the latest medical literature. According to MEDLINE®, the U.S. National Library of Medicine, between 2,000 and 4,000 citations have been added to its database each day since 2005.

Zynx Health's Knowledge Analyzer is a web-based tool designed to help organizations quickly realize the full value of their electronic health record by analyzing vast amounts of clinical content against the most current evidence–bringing added transparency to clinical practice. It will be shown in conjunction with Consensus Builder, the just-released addition to Knowledge Analyzer that helps clinicians quickly reach agreement on the best evidence-based practices to use for optimal patient care.

Consensus Builder provides clinicians with an online tool that aggregates all opinions and discussions related to the clinical content analysis in one location, making it easier for colleagues in different departments or geographic locations to view the entire conversation, offer suggestions, and ultimately vote on their preference.

ZynxCare cancer content module treats the whole patient

Although ample evidence-based content exists relating to the specifics of treating different types of cancer, finding information on how to care for the needs of patients with cancer in general can be far more challenging. Zynx Health's new ZynxCare module offers an interdisciplinary approach to suggesting activities and treatments to address issues that result from cancer and cancer treatment, such as symptoms arising from radiation or chemotherapy treatments.

Moreover, the module's evidence-based recommendations assist patients with pain management, treatment-related problems (such as esophageal irritation and psychosocial issues) to help patients become more engaged in their own healthcare. Holistic and alternative medicine options (such as yoga) also are part of the recommendations. Providers can compare their current plans of care against this module, review which alternatives have proven effective for others, determine areas for improvement, and make adjustments accordingly.
