Campbell will describe how one accountable care organization (ACO) in Northern California deployed a shared mobile care coordination communication platform powered by ZynxCarebook™ to improve its ability to support transition needs of patients in moving from acute care to post-acute settings. The ACO realized a 40 percent increase in post-discharge referrals to case management, 30 percent increase in post-discharge appointments kept, 39 percent increase in medication discrepancy, and a 10.2 percent readmission rate (compared to Medicare average of 21 percent).
Orr and Herzer will co-present Baylor Scott & White Health's plan to update and simplify its electronic care plan templates to be more concise, meaningful and evidence-based via the ZynxCare® solution. The 18-month process at the Temple, Texas-based healthcare organization resulted in a reduction of redundant problems and interventions, and minimized verbiage and decreased mouse clicks that improved user satisfaction and compliance.
