Evolent and Georgia Physicians for Accountable Care establish partnership to improve care delivery by ensuring high quality while enhancing the experience for patients and providers
WASHINGTON, May 12, 2016 /PRNewswire/ -- Evolent Health, a company providing an integrated value-based care platform to the nation's leading health systems and physician organizations, announced today its partnership with Georgia Physicians for Accountable Care (GPAC). The partnership will guide the development and execution of a value-based care strategy for improving patient health in Georgia.
