Health Endeavors Integrates Automated Patient Take Action (APT) Technology with Patient Health Integrated Tools (PHIT) Platform

PHOENIX, July 15, 2015 /PRNewswire/ -- Health Endeavors announces the release of its Automated Patient Take Action (APT) technology.  APT is an advanced technology that utilizes Claims, EMR, Lab and Diagnostic data integrated into the Health Endeavors Data Warehouse Central Repository ("Data Sources") to populate patient disease and wellness gaps in care.  The result of this data integration is APT.  APT will utilize all available Data Sources to automatically prompt patients, care managers, staff and providers via secure and encrypted alerts, emails or text messages to "Take Action."  APT has been incorporated into Health Endeavors cloud-based Care Coordination Tool (CCT).
