New Data: 75% Lower Rate of Major Injury from Falls with Nurse-Led, AI Predictive Care Model

Model helps personal care teams flag risk periods and intervene with staff coverage.

PHILADELPHIA, Sept. 23, 2026 /PRNewswire/ -- BAYADA Home Health Care, a leading nonprofit home health care provider, today announced new data suggesting that falls are 75% less likely to result in major injury when a BAYADA Home Health Aide is present. The finding comes from BAYADA's Enhanced Quality of Care model that helps older adults stay safe and well at home using 40+ client datapoints and predictive AI technology to inform nurse-led care coordination.

Introduced nearly a year ago, the care model now supports about 8,000 older adults nationwide who receive BAYADA's private-pay personal care services.

Why This Matters

Falls are the leading cause of injury among older adults, and when a fall results in major injury, the consequences compound quickly.

One in four older adults report falling every year, and the average inpatient cost of a fall-related hospitalization is nearly $19,000. Such injuries can be followed by a recovery period and complications associated with a lasting loss of independence, even death.

Fear of falling can cause older adults to limit their activity and social engagement, accelerating the very decline they want to prevent. Reducing the severity of harm when falls occur is not only a clinical goal; it is central to preserving older adults' independence and quality of life.

Data Show Presence Reduces the Severity of Harm

Since adopting this care model, BAYADA has observed a directional decline in falls with major injury—a significant drop of about one major injury per week and 58 major injuries per year among this older adult population.

Improved Outcomes Without Increased Spending

For an industry facing chronic workforce shortages and cost pressures, the most compelling implication may be the real possibility of improving outcomes without increasing spending.

BAYADA's AI-predictive technology helps nurse clinical managers identify an individual's high-risk periods—getting up in the morning, bathing, or other routine moments where a fall has occurred before—and supports staffing those hours. The goal is not more care hours, but the right care hours.

A Story of Client Success

In the six months before Debra Chapman initiated personal care services at home, she fell 26 times—so often, that police and EMTs became familiar responders. Those falls were traumatic and sometimes led to inpatient hospitalization. In the past year, with BAYADA Certified Home Health Aide Felicia Washington caring for her, Debra has fallen only twice—both times, without injury.

BAYADA is partnering with payers to develop a similar risk model for Medicaid clients, bringing this nurse-led, data-informed approach to a broader population. Meanwhile, BAYADA continues to study and refine its existing Enhanced Quality of Care model in collaboration with third-party researchers and technology partner, AlayaCare.

Setting a New Bar for an Unmeasured Industry

Home care has no accepted industry benchmark for falls or fall-related harm. In its absence, BAYADA is measuring outcomes against its own historical performance—building a discipline of continuous improvement where the industry has not yet defined a standard.

Falls are one of the first areas where BAYADA has structured outcomes data, and this early signal points to the model's broader potential—though the company is careful to note the statistics are preliminary and not yet independently validated.
