Pair Team Expands to Nevada to Deliver Whole-Person Care for High-Risk Medicaid Members
Pair Team expands to support Clark County's Medicaid population with medical, behavioral, and social care services
CLARK COUNTY, Nev., Sept. 10, 2025 /PRNewswire/ -- Pair Team, a tech-enabled medical group reimagining how Medicaid supports the most underserved populations, today announced its expansion to Nevada. The organization will begin serving high-risk Medicaid members in Clark County as part of its mission to improve access in communities and populations that face persistent barriers to care. Pair Team partners with trusted local organizations to build Community Care Hubs, which are networks rooted in the community that connect individuals to medical, behavioral, and social support services.
Clark County is home to nearly three-quarters of Nevada's population and more than 800,000 Medicaid enrollees. Yet many residents, particularly those with complex medical and social needs, still lack access to consistent, high-quality care. Pair Team's model aims to address that gap by offering comprehensive medical, behavioral, and social services—often starting with the most critical needs such as help with housing, food, and transportation.
Beginning September 2, eligible Medicaid members attributed to Pair Team began receiving personalized outreach and engagement to begin care. Members can meet with their care team by phone, video, or in their homes or communities – wherever care is most accessible.

Pair Team care teams are led by Community Health Workers (CHWs) and supported by nurse practitioners, registered nurses, licensed clinical social workers, and behavioral health specialists. The teams are embedded in the community and work in partnership with local organizations like shelters, food banks, and clinics to ensure members receive personalized support across all aspects of their health.

The expansion builds on Pair Team's success in California, where the company currently serves over 15,000 high-need Medicaid members. Results to date include:
- 52% reduction in emergency department visits
- 26% reduction in inpatient admissions
- 21% increase in outpatient care
- 20% cost savings among high-cost Medicaid beneficiaries
- $25,000 average annual net savings for high-cost dual eligibles
