Primary Partners Alliance Demonstrates Nearly $70 Lower PMPM Healthcare Costs Using Lightbeam Health Solutions

Analysis highlights lower costs and utilization, fewer readmissions and higher HCC recapture through an integrated population health strategy

A one-year analysis of PPA's Medicare Shared Savings Program (MSSP) and ACO REACH population found that patients that PPA successfully identified, engaged, and supported through a completion of an Annual Wellness Visit (AWV) consistently demonstrated better outcomes than those who did not complete an AWV. The findings reinforce PPA's approach of using AWVs as an entry point to ongoing patient engagement and care management.

Key findings included:

• Nearly $70 lower healthcare costs PMPM ($947 vs. $1,014)
• 44% lower readmission rates (9% vs. 16%)
• Lower emergency department utilization (272/K vs. 286/K)
• Higher HCC recapture (70% vs. 57%)
