Delivered through the same RadiantGraph platform that connects pharmacy, benefits, and preventative care outreach, Care Guidance Agents extend support into the ongoing coaching, education, and adherence work that has traditionally required a human care manager.
Healthcare experts recognize that consistent, personalized, condition specific outreach improves outcomes for members with these conditions. But sustained, personalized support is difficult to deliver at scale due to the expensive and labor-intensive work needed to do so. Even when built internally, most plans can reach only a fraction of the members who could benefit. Care Guidance Agents are designed to close that gap in personalization by providing condition-specific coaching to every member that needs it.
CMS has also recognized the opportunity to improve care for people with chronic conditions, recently launching ACCESS, a 10-year Medicare program designed to accelerate the adoption of digital and AI-enabled care. Since RadiantGraph is participating in the program, providers can enroll their Medicare Part B patients to use RadiantGraph's MSK or Behavioral Health agents. Enrollment gives providers an opportunity to deliver improved, consistent patient care while also qualifying for new reimbursement streams.
Care Guidance Agents reach members via voice in four key treatment moments: post-diagnosis education that reduces early confusion and improves follow-through on care plans; daily management and lifestyle coaching tailored to members' real-life constraints; medication and care plan adherence support; and preventive care support that helps members complete required labs and screenings before unseen issues become costly events.
