134 United Physicians Practices Designated As Blue Cross Blue Shield Of Michigan Patient-Centered Medical Homes

BINGHAM FARMS, Mich., July 24, 2014 /PRNewswire-USNewswire/ -- United Physicians announced today that Blue Cross Blue Shield of Michigan has designated 134 United Physicians primary care practices representing 360 primary care physicians (PCPs) as Patient-Centered Medical Homes (PCMH). The designation period runs from July 1, 2014 through June 30, 2015.

To become a PCMH designated practice, primary care physicians lead a professional healthcare team that utilizes health information technology and administrative processes to deliver more coordinated, efficient, and effective medicine to their patient population. PCPs are responsible for encouraging their patients to take a more active role in maintaining and improving their own health; monitoring their patients' overall health and wellness; developing individualized health goals; and coordinating their patients' prescriptions, tests and therapies to ensure their health care needs are being met.
